I am trying to add files to a recently created folder that has been added at the root.  I can create new folders and subfolders however if i try to add files to either of these, I get the following error:

Violation of PRIMARY KEY constraint 'PK_dnn_DMX_EntryPermissions'. Cannot insert duplicate key in object 'dbo.dnn_DMX_EntryPermissions'. The statement has been terminated.

The files are not added and I cannot add the files using WebDAV either.

I can still add files to the existing folders or at the root.

Thanks for your help Peter.  It was exactly as you described with a full duplication of the permission entries (except for differing ids).  We deleted the second set of entries.

I am finding that there are a number of unapproved documents left over from the failed attempts at creating new documents.  They have no title and some have no size.

I cannot delete these from the unapproved list using the hard or soft delete.  Is there another way?
