Try emptying your browser's (temporary files) cache and resetting the menus.

It didn't work but right direction.
I've solved it by clearing 'Enabled Composite files < Client Resource Management< Advanced Setting < Host Setting' and reinstalling the package.

I have the same problem, but the solution proposed by Stas Popov is not working for me.

How could be solved?

Maybe there is a problem in your module.css. This file can be found in the folder Bring2Mind\DMX. Check if you see '06.00.04' en replace it with '06.01.00' (find and replace). That helped for me.

yes, I know... Unfortunately I have both the problems on my installation of DMX... I hope to find out something debugging the css. However, thank you Peter: DMX is really a good product

Both problems? Sorry to hear. The missing icons is most likely due to the wrong sprite in the client browser. This is a result of caching. The sprite changed for this version and the old version is then still being used while the css is new. It is fixed by refreshing the browser cache.

I had reset IE and than reinstalled the package after clearing 'Enabled Composite files < Client Resource Management< Advanced Setting < Host Setting'. To be fair shouldnt be any difference because installation package unpacked on the server side anyway but it works for me.
